This is the one I bought off of ebay:

well about your "output adapter," i don't know exactly what you bought because i cant find anything online that matches what you state, but I assume it is a line level converter. what you have to do is go behind your stock radio, locate the rear speaker leads (4 total; LR+, LR-, RR+, RR-) and tap into those wires. usually on the converter it is 2 gray wires, 2 white wires, and a black wire. the black wire you shouldn't have to worry about. Now that you have located all your wires, the LR+ wire from the radio goes to the solid white wire on the converter (LR+), and the LR- wire from the radio goes to the white/black wire on the converter (LR-). Then, the RR+ wire from the radio goes to the solid gray wire from the converter (RR+), and the RR- wire from the radio goes to the gray/black wire from the converter (RR-). Than plug in your RCAs to the converter and run them as you normally would. ***Remember, RCA and power wire down OPPOSITE sides of the car!!***
and just in case:
LR+ = left rear positive speaker wire
LR- = left rear negative speaker wire
RR+ = right rear positive speaker wire
RR- = right rear negative speaker wire
Also, depending on the converter you got, the wire colors could be different. just follow what i wrote using the LR's and RR's in parentheses and you'll be fine.
good luck
